Cushing’s syndrome, also known as hypercortisolism, is a disease caused by excessive secretion of glucocorticoids from the adrenal cortex.
It is a rather rare condition, with an average incidence of two to three cases per million people.
Among those affected, the ratio of women to men is three to one.
Its symptoms are varied, with the main ones being centripetal obesity, a moon-shaped face, weight gain, purple stretch marks on the skin, hypertension, and abnormal sex hormone levels, among others.
In Xia Min’s case, now that the cause was found, treatment would be much simpler.
First, a laparoscopic resection of the left adrenal tumor would be performed. With subsequent medication and physical therapy, it was estimated that Xia Min’s figure would return to normal within two to three months.
Today was Tuesday. At eight in the morning, Yan Feifan arrived at the Neurosurgery Department to report to Director Lo Jun.
Lo Jun first asked a few questions about the surgery during his trip to Jingcheng, then followed up by asking about his progress with the books he had given him last time.
Upon learning that Yan Feifan had already finished one, Lo Jun, with great interest, posed a few questions from the book to test him.
The more questions Lo Jun asked, the more astounded he became.
He was utterly amazed when he learned that Yan Feifan had memorized the nearly five-hundred-page professional book from beginning to end, word for word.
Yan Feifan humbly stated that for now, he had only crammed the information into his head and that he still needed much more practice and exploration to truly master it and apply it in clinical practice.
To Lo Jun’s ears, this sounded like blatant showing off.
For medical students, the hardest part is memorization.
Medical students spend at least eighty percent of their time outside of class memorizing countless definitions, key points, operational standards, precautions, and more.
Therefore, during today’s rounds, Lo Jun singled out Yan Feifan and relentlessly questioned him.
The result was that for five patients with different conditions, Lo Jun asked a total of eleven questions, and Yan Feifan answered every single one of them perfectly.
This left Lo Jun feeling a bit frustrated.
He had planned to use any wrong or unanswerable questions as an opportunity to lecture Yan Feifan a bit, to keep him humble and patient.
The problem was, Yan Feifan never gave him that chance.
Under these circumstances, the other doctors on rounds with them were the unlucky ones.
The not-so-pleased Lo Jun periodically caught them making mistakes, big or small, and unleashed a torrent of criticism.
Lo Jun hoped that their ordeal would serve as a warning to the overly clever monkey that was Yan Feifan…
A little after eleven in the morning, just as Lo Jun finished up with some departmental administrative tasks, he saw Yan Feifan knock and enter his office.
“Sir, this is for you!”
Lo Jun took it and saw it was a cashier’s check for 300,000 yuan, issued by the hospital’s research fund management office.
He understood immediately.
“My monthly salary, bonuses, and various allowances add up to tens of thousands.”
He handed the check back with a light chuckle. “I don’t need the money. You’re the one who needs it right now.”
“Take it back.”
Yan Feifan wasn’t one to be overly polite in such matters.
He didn’t refuse again, immediately taking it back. “Thank you for your consideration, sir. I also prepared some tea and red wine. I’ll bring them to you tomorrow.”
Lo Jun couldn’t help but ask, “Does this mean if I had taken the check, I wouldn’t be getting the tea and wine?”
Yan Feifan nodded with complete honesty.
Lo Jun burst out laughing. “You! It’s a good thing you chose to be a doctor. If you were out trying to make it in the world, who knows how badly you’d be beaten down.”
He waved his hand again. “Go and prepare well for this afternoon’s arm nerve transplant surgery. This time, you’ll be the lead surgeon.”
“Yes, sir.”
Yan Feifan acknowledged the order and left the office.
The patient for this afternoon’s surgery was a dancer.
She had injured the ulnar nerve in her left elbow in an accident.
This resulted in paralysis of her interosseous muscles, the ulnar-side lumbricals, and the adductor pollicis, leading to the formation of a claw hand.
The patient’s fine motor skills in that hand were also severely limited, and she had lost all sensation in her little finger.
An examination revealed that the ulnar nerve in her left elbow was severed, and the distal end of the nerve had atrophied and degenerated.
Only a nerve transplant offered a chance of restoring function to her left hand.
For Yan Feifan, this surgery wasn’t much of a challenge.
After all, nerve grafting was a key step in limb replantation surgery.
Just as Mr. Qin had said before, once you’ve fully mastered the main procedures in trauma surgery, you can pretty much swagger through at least half of the other surgical fields.
At twelve noon, Yan Feifan was getting ready to go for lunch.
Just as he left the Neurosurgery Department, he saw Chen Yu, Xia Yunxiao, Xia Min, and a middle-aged couple—one stout, one thin—coming toward him.
After a round of enthusiastic thanks, Chen Yu had the others leave and stayed behind by herself.
She raised her right hand and wiggled her fingers in front of Yan Feifan’s eyes.
“The hand is the most flexible part of the human body, and also the most easily injured.”
“Every year in our country, tens of thousands of people suffer from limited hand function due to tendon injuries.”
“I admit it, that Qin Ying, she’s better than me when it comes to finger replantation surgery. But when it comes to handling all sorts of tendon injuries…”
Chen Yu said proudly, “Even if she were on a champion racehorse, she couldn’t catch up to me.”
Privately, Yan Feifan disagreed with her claim.
At the level of doctors like Mr. Qin and Mrs. Chen, everyone had their own specialties and areas of excellence.
But the difference in their advantages was like someone who ran ninety-five steps versus a hundred—not a huge gap.
Naturally, Yan Feifan only voiced his disagreement in his head; he wouldn’t be foolish enough to question her to her face.
He stood there obediently and listened as Chen Yu continued, “Feifan, to thank you for pinpointing the root of Xiaomin’s obesity…”
“I’m going to teach you all the experience and techniques I’ve accumulated over many years for treating various hand tendon injuries.”
“Thank you for your generous tutelage, Mrs. Chen!” Yan Feifan said, his face full of joy.
Chen Yu gave a light “hmph,” then took out her phone, swiped the screen a few times, and showed it to Yan Feifan.
On the screen was a photo of a young girl with a brilliant smile.
“This is a casual photo of Xiaomin before she gained the weight. It’s a completely unfiltered, no-makeup photo.”
“What do you think? Isn’t she beautiful?”
Yan Feifan nodded, but then it dawned on him. He made an excuse that his sister was waiting for him for lunch and beat a hasty retreat.
This left Chen Yu both exasperated and amused.
“That little rascal…”
The afternoon’s ulnar nerve transplant surgery began at 2:10 PM and concluded at 4:00 PM.
When Yan Feifan came out of the operating room, he was surprised to find an acquaintance waiting for him: Zheng Libin from the district’s Public Security Sub-Bureau.
“Mr. Zheng, are you with the patient?”
Zheng Libin replied softly, “Mr. Yan, I came for you.”
“There’s a case… I was hoping you could help me with something…”
